ABSTRACT

Objective:To evaluate the measurement agreement of Roche 25(OH)D immunoassay(evaluation method) with LC-MS/MS (reference method).Methods:A total of 909 residual serum samples from routine health check participants were collected from May to June in 2019. 25(OH)D concentrations were measured by evaluation method and LC-MS/MS, respectively. Passing-bablok regression, intraclass correlation coefficient (ICC), Bland Altman plots and Kappa test were used to analyze the consistency and bias on the results derived from the two measurement methods.Results:The 25(OH)D concentration derived from evaluation method was significantly different from those from LC-MS/MS method ( P<0.001). Slope of regression for evaluation method and LC-MS/MS was 0.962(95% CI 0.919-1.007), while intercept was -0.185 (95% CI -1.191-0.745). The ICC was 0.765 (95% CI 0.735-0.792). Altman plot showed that the average deviation between evaluation method and LC-MS/MS was -0.902 ng/ml (0.300%). The coincidence rate of evaluation method′s judgment of vitamin D sufficiency, insufficiency and deficiency with LC-MS/MS was 83.39%, and the weighted Kappa values was 0.790. Conclusion:Roche automatic 25(OH)D immunoassay shows acceptable correlation and agreement with LC-MS/MS, however, it is to note that the deviation between immunoassay and LC-MS/MS may lead to wrong judgment of vitamin D nutritional status. It is recommended that each laboratory should establish own corresponding reference values for 25(OH)D concentrations derived from these two methods.

ABSTRACT

Objective To investigate the clonal relatedness of A.baumannii isolates from senile patients by conducting cross-sectional and longitudinal studies on antimicrobial susceptibility profiling and genomic diversity.Methods Cross-sectional study was done among the 170 non-repetitive A.baumannii isolates which were collected from senile patients during two years.Longitudinal study was conducted among 77 A.baumannii collected from 8 senile patients within longtime hospitalization.Results 75.3 % of the 170 isolates were non-susceptible to carbapenems,and the phenotype were XDR or MDR which spread evenly all over the senile wards.The isolates belonged to 36 pulsotypes determined by PFGE.Groups Ⅰ (contain119 isolates) were major epidemic strains,which were CRAB with XDR phenotype.In longitudinal study,comparison of pulsotypes was performed for each patient and all isolates were clustered into group Ⅰ except one isolate.All the 77 isolates were XDR.Conclusion Extensive drug-resistance of A.baumannii was a serious problem in the gerontal wards.Clone dissemina tion was the most important style of XDR strains spread.Horizontal infection control measures to interrupt person-to-person transmission should be reinforced to reduce the further spread of XDR A.baumannii.

ABSTRACT

Objective To quantify the risk factors for aspirin resistance so as to increase the prognosis for risk of coronary heart disease,and to establish a predictive model for aspirin resistance in order to guide the clinical anti-platelet therapy.Methods A total of 938 elderly male patients with stable coronary heart disease (CHD) receiving oral aspirin therapy (>75 mg/d) over 2 months were included in this study.Their clinical data were collected.Logistic regression analysis was performed to establish a predictive model and risk score for aspirin resistance.Hosmer Lemeshow (H-L) test and an area under the receiver operating characteristic (ROC) curve (the area under the ROC curve) were performed to test the calibration and discrimination of the model.Results Seven risk factors were included in the predictive model,including serum creatinine (>110 μmol/L:score of 1),fasting blood glucose (>7.0 mmol/L:score of 1),hyperlipidemia (score of 1),number of coronary arteries in lesion (2 branches:score of 2,≥≥3 branches:score of 4),body mass index[(20-25) kg/m2:score of 2,>25 kg/m2:score of 4],percutaneous coronary intervention (score of 2),smoking (score of 3).H-L test showed P≥0.05 and the area under the ROC curve>0.70 in this model.Conclusions the risk factors for aspirin resistance,and establishing a valid predictive model for aspirin resistance,could provide an important reference for anti-platelet therapy in CHD patients.

ABSTRACT

Objective To evaluate a new platelet function analyzer PL-11 with three major platelet function methods.Methods A randomized controlled trial was adopted.Totally 20 healthy young students from People's Liberation Army Medical School were enrolled in the study during July and August in 2013.Subjects were treated with aspirin or clopidogrel and the platelet function was measured by using of PL-11,as well as light transmittance aggregometry (LTA),VerifyNow and thromboelastography (TEG).Results When monitor aspirin response,correlations between arachidonic acid (AA) induced PL-11 and other methods were:LTA,0.614; VerifyNow,0.829; TEG,0.697,respectively.Biases between AA induced PL-1 1 and LTA were 1.94% at baseline and 24.02% during aspirin treatment.Cut-off value of aspirin response tested with AA induced PL-11 was 33.3%.When monitor clopidogrel response,correlations between adenosine piphosphate (ADP) induced PL-11 and other methods were:LTA,0.767; VerifyNow,0.851; TEG,0.608.Biases between ADP induced PL-11 and LTA were 3.01% at baseline and 6.56% during clopidogrel therapy.Cut-off value of clopidogrel response tested with ADP induced PL-1 1 was 66%.Conclusion Results of different platelet function testing methods were not equally effective.PL-11 has a high capability when monitoring short aspirin or clopidogrel response.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the clinical manifestations and hematologic parameters and observe the cytomorphological features of metastatic tumors in the bone marrow originating from different primary sites.</p><p><b>METHODS</b>The clinical data of 77 patients with bone marrow metasta tumors admitted between 2009 and 2014 between 2009 and 2014 in General Hospital of PLA were studied retrospectively to analyze the indications of laboratory examinations (hematological laboratory tests, tumor markers, peripheral blood films, and bone marrow aspirates).</p><p><b>RESULTS</b>Of the 77 patients analyzed, 64.9% were over 50 years of age. The most common clinical characteristics were bone pain (65%), anemia with thrombocytopenia (63.6%) and leukoerythroblastic reaction (61%). The hematological abnormalities included elevation of ESR, ALP, LDH, tumor markers, and hypoproteinemia. Cytological examination of bone marrow aspiration samples revealed different morphological characteristics of the metastatic cells from different primary sites; in most of the cases, scattered or clustered metastatic cells and degenerative tumor cells were found on the edge of the bone marrow smears.</p><p><b>CONCLUSION</b>Detection of the primary tumor site is difficult by cytological examination of bone marrow aspiration samples, but the cytological findings can be of value in the diagnosis of neuroblastoma, small cell lung cancer and gastric cancer (signet ring cell carcinoma). A definite diagnosis of bone marrow metastatic tumor relies on a combined evaluation of the disease history, clinical symptoms and laboratory findings.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To assess the association of vitamin D receptor (VDR) gene Fok I and Bsm I polymorphisms with dyslipidemia in elderly male patients with type 2 diabetes of Han nationality.</p><p><b>METHODS</b>A total of 328 elderly male residents of Han nationality in Beijing, including 237 type 2 diabetic patients and 91 healthy control subjects, were enrolled in this study. The diabetic patients were divided into non-dyslipidemia group (DO group, n=134) and dyslipidemia group (DH group, n=103). All the participants were genotyped for Fok I and Bsm I polymorphisms in VDR gene using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and DNA sequencing technology, and the results were compared with their clinical characteristics.</p><p><b>RESULTS</b>For Fok I, the frequency of F allele was significantly higher in the diabetic patients than in the control group (Χ(2)=3.873, P=0.049, OR=1.439, 95% CI: 1.001-2.071). In the dominant model, the frequency of FF genotype was significantly higher in the diabetic group (Χ(2)=5.057, P=0.025, OR=1.756, 95% CI: 1.072-2.875) as well as in DH group (Χ(2)=6.168, P=0.013, OR=2.06, 95% CI: 1.161-3.663) than in the control group. There was no significant differences in the genotype frequency or allele distribution in other paired groups (P>0.05). Compared with Ff + ff genotype, FF genotype was associated with a significantly decreased average diastolic blood pressure (P=0.039) but significantly increased postprandial blood glucose (P=0.035), triglycerides (P=0.049) and uric acid (P=0.031). No significant difference was detected in genotype frequency or allele distribution of Bsm I polymorphisms between the groups (P>0.05); serum creatinine levels were significantly higher in bb genotype than in BB + Bb genotype group (P=0.011).</p><p><b>CONCLUSION</b>VDR gene Fok I polymorphisms may be a risk factor for dyslipidemia in elderly male patients with type 2 diabetes among Chinese Han population, where Bsm I polymorphisms are not associated with diabetic dyslipdiemia.</p>

ABSTRACT

Objective To investigate the changes of sex hormone and androgen receptor levels and evaluate the relationship of the sex hormones and androgen receptor with coronary heart disease (CHD) in elderly men. Methods A cross-sectional study was performed in 539 elderly men, including 400 healthy people aged 62-92 years and 139 CHD patients aged 60-88 years. The plasma concentrations of total testosterone (TT), free testosterone (FT), dehydroepiandrosterone sulfate (DHEAS), sex hormone binding globulin (SHBG), estradiol (E2), luteinizing hormone (LH) and follicle-stimulating hormone (FSH) were measured. The androgen receptor (AR) level was tested by flow cytometry. Results The fluorescence intensity of DHEAS, TT, SHBG, FT and AR were significantly lower in CHD group than in healthy group (P<0.01);however, FSH and E2 in CHD group were higher(P(0.01). Age was negatively correlated with TT(r=-0.28,P<0.01) and FT (r=-0.17,P<0.05), and positively correlated with SHBG(r=0.14,P<0.05) and E2 (r=0.33, P<0.01). AR fluorescence intensity was negatively correlated with systolic blood pressure (r=-0.12,P<0.01). Logistic regression analysis indicated that TT (OR=1.065,9% CI: 1.012~1.121,P<0.05), SHBG(OR=0.994,95% CI:0.990~0.998,P<0.01) and AR (OR=0.971,95%CI:0.956~0.986, P<0.01)were significantly associated with CHD in elderly male patients. Conclusions The levels of DHEAS, TT, SHBG, FT and AR are lower in elderly men with CHD than in elderly healthy men;however, the FSH and E2 concentrations are higher. Low levels of TT, SHBG and AR may be the independent risk factors for CHD in elderly men.

ABSTRACT

Objective: To investigate the effects of traditional Chinese medicine Baoyuanhuoxue Granula on the state of senile type Ⅱdiabetes before thrombus.Methods: 60 cases of selected senile type Ⅱdiabetes patients were divided into two groups,trial group(treatment with routine western medicine added Baoyuanhuoxue Granula;30 cases) and control group(treatment with simple western medicine; 30 cases).After two months,the changes of expression rate of blood viscosity, nail fold microcirculation and platelet aggregation,D-D,CD-62p and PAC-1 of patients in two groups before and after treatment were observed.Results: Baoyuanhuoxue Granula can reduce the expression rate of blood viscosity,platelet aggregation,D-D,CD-62p and PAC-1 obviously,and can improve nail fold microcirculation.Conclusion: Baoyuanhuoxue Granula can improve the state of senile type Ⅱdiabetes before thrombus.

Objective To approach the dynamic change in the membrane protein on platelet microparticles of patient with anti-platelet therapy, and look for a new test which are helpful to supervise the anti-platelet therapy. Method We collected blood samples from the patient with myocardial infarction ( n =9) treated with ticlopidine (250 mg?2 in the first and second day, 250 mg/day from the third to sixth day)in different time (before treatment, and 4 d, 5 d, 6 d, 9 d, 10 d after treatment), and examine the percent of PAC-1 + PMP or CD62p + PMP in the samples by activating platelet with 20 ?mol ADP. Result By activating platelet with 20 ?mol ADP, the percent of CD62p + PMP at different time (before treatment, and 4 d, 5 d, 6 d, 9 d, 10 d after treatment) were 84.3%?3.6%, 81.4%?3.4%, 70.3%?2.7%, 70.6%?3.2%, 83.5%?2.8%, 85.4%?2.1%,as of PAC-1 + PMP were 85.3%?3.5%, 82.5%?2.2%, 72.3%?3.5%, 72.4%?3.3%, 85.4?3.4%, 86.0%?3.8%. ABSTRACT

Objective To observe the Ca 2+ concentration changes in the platelets activated by chemical agonist in different study group treated with aspirin.Methods We measured the Ca 2+concentration in the platelets activated by different concentration of ADP, thrombin, collagen, arachidonic acid and 5-HT with the flowcytometer in normal health (n=20).We measured the Ca 2+ concentration in the platelets activated by ADP, collagen, arachidonic acid and 5-HT in myocardial infarction (n=20) and hypertension (n=20).We measured the Ca 2+concentration in the platelets activated by arachidonic acid in myocardial infarction (n=20) treated with aspirin.Results As the concentrations of ADP, thrombin, collagen, arachidonic acid and 5-HT increased, the Ca 2+concentration in the platelets increased.There was the relation between agonist concentration and Ca 2+ concentration.Activated by the same agonist, the Ca 2+concentration in the platelets showed more increase in myocardial infarction and hypertension than in normal health.The Ca 2+ concentration show less increase in myocardial infarction treated with aspirin than in normal health.Conclusion The procedure which chemical agonist activating platelets is related to Ca 2+ concentration.The activity of Ca 2+ channels in platelets is high in myocardial infarction and hypertension.Examining the Ca 2+ concentration in platelets is helpful to supervise the aspirin therapy in myocardial infarction.

ABSTRACT

Objective To investigate injecting recombinant human granulocyte-colony stimulating factor(rhG-CSF) on the proportion of peripheral neutrophilic granulocyte and their surface molecules expression in healthy people.Methods The peripheral blood samples were collected from the healthy people who were injected with rhG-CSF.The expressions of CD62L,CD11a,CD44 and CD49d on the surface neutrophilic granulocytes were determined before and after injection with flow cytometery.Results The median percentage of neutrophilic granulocytes in peripheral blood leucocytes after injecting rhG-CSF significantly increased from 60% to 85%(P0.10).Conclusion Injecting rhG-CSF may obviously impact the expression of CD62L and CD49d on neutrophilic granulocytes.
